Benefit Limits
Similar to previous years, drug-specific annual deductibles, maximum annual benefits, or out-of-pocket limits are not included in the vast majority of employers' plan designs. In 2011, 22% of respondents had a drug-specific annual deductible, 8% had a maximum annual benefit, and 14% offered a drug-specific out-of-pocket limit.
